Understanding Native Speakers

Why is it still difficult to understand native speakers after several months of language learning? Even if you actively learn words and grammar, understanding spoken language remains a big problem. What could be the reason: lack of practice, incorrect learning methods, or individual perception? What specific exercises can help improve listening comprehension?
